Patients/Family Members/Legal Guardians
Please download, print out and complete a Patient Request Form with photocopies of a valid driver license/state ID with current address.

If requesting a record on behalf of a minor or family member, please print out and complete a Patient Representative Request with copy of driver license/state ID with current address and any of the following documentation:

• copy of birth certificate of patient who is a minor stating showing representative is the parent
• copy of death certificate of deceased patient noting representative as next of kin
• power of attorney or other legal documentation as acceptable proof of legal right to act on behalf of patient

• Formal request on company letterhead with patient’s full name, date of birth, date of incident and address/location of incident and a signed HIPPA form authorizing release of patient information to requesting law firm or legal service.
  
• OR, a Deposition Subpoena for Production of Business Records with an accompanying "Notice to Consumer or Employers" and a Declaration of Custodian of Records.
  
• Processing fee of $15.00 per patient for copies of paramedic reports/billing. Checks should be payable to "Rural/Metro San Diego."
